Care When and Where You Need It

Vanderbilt Health Nurse Navigation, which began a pilot this summer with Metro Nashville Public Schools, offers a faster, more convenient way for employees to get care when and where they need it throughout the Vanderbilt Health System.

The high-touch, nurse-navigated model supports the physical, mental, and spiritual health of MNPS teachers and their dependents. A dedicated nurse navigator and licensed clinical social worker pair with Vanderbilt Health On Call nurse practitioners, Palliative Care team and Home Health team to deliver care.

The program’s holistic approach includes both a mental and spiritual health component to meet members where they are. Same-day telehealth appointments, clinic visits, support groups and an educational series are available.
